In order to provide a safe and secure environment for the children and teens who attend our church and related events, we believe it is necessary to ask the following questions as part of your application process. All the information you provide will be kept in confidence by the church leadership and will not be disclosed by the church unless required by law. Answering yes to any of the following questions may not necessarily prevent you from volunteering with the church. Thank you in advance for your understanding.

By checking the box below, I hereby give Bayfair Baptist Church consent to verify the information provided by me in this form and to contact references (if asked for. References will only be requested if considered necessary at the discretion of the Ministry Lead, Church Staff, or Elders), current and former pastor, or church ministry personnel of churches listed above to obtain and verify any information from them (and anyone else the church determines might be able to provide relevant information) that may be relevant to this application.

I grant permission for the church to perform a police check on me.

I further grant the church permission to perform an internet search on me and to review and consider any information found there.

I understand that if the church approves my volunteer application and later determines, in its discretion, at any time that I am not suitable for serving/volunteering service to the church or the position for which I am volunteering, the church may terminate my volunteer service or position for any reason without advance notice.

If the church approves my application for serving/volunteering, I will sign any documents that the church requires, will at all times cooperate fully with the staff of the church in the fulfillment of my duties, and will keep all confidential information I encounter in my role as a volunteer confidential.

If at any time I determine that for any reason I am unable to support, adhere to, or follow the policies, procedures, or doctrine of the church, I will inform the church and will step out of my volunteer position.
